Pengaruh pemberian patch hidrogel ekstrak daun kelor (Moringa oleifera) 9% terhadap penyembuhan luka akut pada kulit tikus wistar (Rattus norvegicus) jantan berdasarkan ekspresi kolagen dan vascular endothelial growth factor

Abstract

Introduction: Hydrogel as a wound dressing option can be combined with several other active ingredients such as antioxidants found in Moringa oleifera (MO). This combination is expected to accelerate the wound healing process through increased vascularisation and collagenesis. This study aimed to determine the effect of hydrogel in combination with MO extract on epithelial thickness, collagen expression and VEGF in acute wounds of Wistar rats.

Methods: An experimental study involving 36 Wistar rats that divided into 3 groups (positive control (C); hydrogel alone (P1); and hydrogel with 9% MO leaf extract (P2). Wound evaluation was performed on days 1, 7, and 14 by assessing epithelial thickness, collagen expression and VEGF through histopathological examination, hematoxylin eosin staining and immunohistochemistry. Analysis was performed with SPSS program and analyzed using Kruskal-Wallis and Post-hoc test.

Results:  On day 7 evaluation, the mean epithelial thickness of group P2 (54.62±9.31 μm) was found to be significantly lower than the control and P1 (p=0.018). On day 14, epithelial thickness of group P2 (27.97±1.22 μm) significantly lower than the control and P1 groups (p=0.02). Significant differences in collagen expression were found between the three groups on day 14 (p=0.044), where group P2 had significantly higher collagen expression than control (p=0.029) and P1 (p=0.014). The VEGF expression in group P2 was significantly higher than the control on day 7 (p=0.029), but not significantly different on day 14.

Conclusion: The combination of hydrogel and 9% MO extract can provide better wound healing than hydrogel alone and 0.9% NaCl compress.

 

Pendahuluan: Hidrogel sebagai pilihan balutan luka dapat dikombinasikan dengan beberapa bahan aktif lainnya seperti antioksidan yang dapat ditemukan pada Moringa oleifera (MO). Adanya kombinasi ini diharapkan dapat mempercepat proses penyembuhan luka melalui peningkatan vaskularisasi dan kolagenesis. Tujuan penelitian ini adalah untuk mengetahui efek pemberian hidrogel kombinasi ekstrak MO terhadap tebal epitel, ekspresi kolagen dan vascular endothelial growth factor (VEGF) pada luka akut tikus wistar.

Metode: Penelitian eksperimental melibatkan 36 tikus wistar yang terbagi ke dalam 3 kelompok (kontrol positif (K); hidrogel saja (P1); dan hidrogel dengan ekstrak daun MO 9% (P2)). Evaluasi luka dilakukan pada hari ke-1, ke-7, dan ke-14 dengan menilai tebal epitel, ekspresi kolagen, dan VEGF melalui pemeriksaan histopatologi dan pengecatan hematoxylin eosin serta imunohistokimia. Analisis dilakukan dengan program Statistical Package for the Social Sciences (SPSS) dengan uji Kruskal-Wallis dan uji Post-hoc.

Hasil: Pada hari ke-7, didapatkan rerata tebal epitel kelompok P2 (54,62±9,31 μm) signifikan lebih rendah dibandingkan kelompok kontrol dan P1 (p=0,018 μm). Pada hari ke-14 dengan rerata tebal epitel kelompok P2 (27,97±1,22 μm) signifikan lebih rendah dibandingkan kelompok kontrol dan P1 (p=0,02). Perbedaan signifikan pada ekspresi kolagen ditemukan antara ketiga kelompok pada hari ke-14 (p=0,044), ekspresi kolagen kelompok P2 signifikan lebih tinggi dibandingkan kontrol (p=0,029) dan P1 (p=0,014). Peningkatan ekspresi VEGF pada kelompok P2 secara signifikan lebih tinggi dibandingkan kelompok kontrol pada hari ke-7 (p=0,029), namun tidak berbeda secara signifikan pada hari ke-14.

Simpulan: Kombinasi hidrogel dan ekstrak MO 9% dapat memberikan penyembuhan luka yang lebih baik dibandingkan dengan hanya hidrogel saja dan kompres NaCl 0,9%.
