Hubungan riwayat konsumsi alkohol, ukuran varises, derajat sirosis hati terhadap perdarahan varises esofagus pada pasien sirosis hati di RSUP Prof. I Goesti Ngoerah Gde Ngoerah

Abstract

Background: Esophageal varices is a disease characterized by abnormal enlargement of the veins in the esophagus which causes the varices to rupture and esophageal variceal bleeding occurs. Bleeding esophageal varices is one of the main causes of death in liver cirrhosis patients. Several risk factors for esophageal variceal bleeding in liver cirrhosis are the presence of large varices, a high Child Class Cirrhosis Score, excessive alcohol consumption. This study aims to determine the relationship between alcohol consumption, varicose size, degree of liver cirrhosis and esophageal variceal bleeding in liver cirrhosis.

Methods: We conducted an analytical observational study that uses a cross-sectional research design. The total sampling technique was used as a method for determining the number of samples. The data was then analyzed using SPSS.

Results: The results of the study showed that patients with liver cirrhosis with esophageal variceal bleeding were dominated by grade III varicose size, 35 patients (94.6%) and based on the results of the Chi Square test, the p value was 0.001, while for the degree of liver cirrhosis, the P value was 0.004, and alcohol consumption obtained a p value of 1,000.

Conclusion: The size of esophageal varices and the degree of liver cirrhosis were related to the incidence of esophageal variceal bleeding in patients with liver cirrhosis. Meanwhile, alcohol consumption is not related to the incidence of esophageal variceal bleeding in liver cirrhosis patients.

 

Latar belakang: Varises esofagus merupakan penyakit yang ditandai dengan adanya pembesaran abnormal pembuluh darah vena di esofagus yang dapat menyebabkan varises dapat pecah dan terjadi perdarahan varises esofagus. Perdarahan varises esofagus merupakan salah satu penyebab utama kematian pasien sirosis hati. Beberapa faktor risiko perdarahan varises esofagus pada sirosis hati adalah adanya ukuran varises besar, skor Child class sirosis yang tinggi, konsumsi alkohol yang berlebihan. Penelitian ini mengevaluasi hubungan konsumsi alkohol, ukuran varises, derajat sirosis hati dengan perdarahan varises esofagus pada sirosis hati.

Metode: Kami melakukan penelitian observasional analitik melalui pendekatan potong lintang. Teknik total sampling digunakan sebagai metode menentukan jumlah sampel. Data kemudian dianalisis menggunakan SPSS.

Hasil: Hasil penelitian menunjukkan bahwa pasien sirosis hati dengan perdarahan varises esofagus didominasi oleh ukuran varises grade III sebanyak 35 pasien (94,6 %) dan berdasarkan hasil uji Chi Square didapatkan P value 0,001, sedangkan pada derajat sirosis hati didapatkan P value 0,004, dan konsumsi alkohol didapatkan P value 1.000.

Kesimpulan: Hasil evaluasi melaporkan bahwa ukuran varises esofagus dan derajat sirosis hati berhubungan terhadap kejadian perdarahan varises esofagus pada pasien sirosis hati. Sedangkan konsumsi alkohol tidak berhubungan terhadap kejadian perdarahan varises esofagus pada pasien sirosis hati.
